I'm thinking that people who have this ability are kept under tight control by this (really repressive) society because they are considered to be dangerous for some reason. Perhaps they have varying degrees of telepathic ability and the people in power are afraid of that, afraid of their own dirty secrets being revealed or plots being hatched against them by those who are able on occasion to read their minds.
The reason some of them are allowed to become doctors could be because it's a "safe" use of that ability.
