I feel like I keep commenting that every chapter is painful, but I have to say it again -- This was painful! But in such a good, real way that the show ignored. I'm really glad you're exploring this 'lost' period.
As for this chapter, I think it's perfectly IC that House would exact physical pain on himself as a distraction from the (worse?) emotional pain. I also 'awwwed' at the idea that he never enjoyed Halloween as a kid. I guess that could have been his nature even then, but I feel like it may be b/c of Dad. (In my head, I've thought that House's fondness for childish games and pranks may be because he wasn't allowed that stuff as a kid.)
I love how Wilson's section illustrates how very alone he is too -- and how attached he still is to House, when he can't help but be angry at the douchebag radiology tech. (I've never fully understood why Wilson is unable to have a lasting relationship outside of House -- it's one of the mysteries of his character.)
